joining us now to talk about all of this,and also the spread of the new covid,sub-variant here in these United States,is Dr Ashish Shah the White House,coveted response coordinator Dr John,good morning thank you for being here,hey good morning thanks for having me,back,so we just heard in the Liz Palmer,report about China that we're not,getting reliable information about the,spread of covet in that country no,surprise there but how concerned are you,that we might get a big surprise in the,form of a more lethal or dangerous,variant from that country,yeah it's a very good question look,first and foremost our hearts go out to,the Chinese people who a lot of whom are,suffering right now because of the way,the virus is spreading in the country in,terms of openness and transparency this,is something we have believed very,strongly from the beginning China should,be forthcoming on the data and on the,issue of a new variant you know one of,the things that China is not doing is,sharing its genomic surveillance data,every other country does this is when,you take the virus and you sequence it,and then you make it publicly available,every other country is doing it China is,not that makes it very hard for the,scientific Community to spot new,variants really important for the,Chinese government to do that so we may,not even know the U.S has imposed,testing requirements for travelers,coming from China and that's raised,questions about xenophobia anti-asian,bias well it will contribute to that,putting that to one side there's also,the question of will it be effective how,effective do you think that will be in,preventing any variant that could be of,concern from coming in from China yeah,so first and foremost on the issue of,you know the policy itself it applies to,everybody whether you're an American,you're a Chinese person if you're coming,from China you need to get a test first,it's part of a comprehensive policy and,the comprehensive policy has testing,before you leave also has enhanced,genomic surveillance once people arrive,here this is our effort to make sure,that if there is a new variant we spot,it early and that we try to keep as many,infections out of the country as,possible I think the whole package,itself is going to be very effective,Dr Shaw how much will the latest booster,shot protect Americans from the new,variant spreading here it's a great,question the answer is substantially,better than any of our prior vaccines,and here's what we know based on all the,data we have the new bivalent vaccine,this is the new one that we updated in,September uh does seem to provide a good,degree of protection against infection,and terrific protection against serious,illness so this is why the number one,thing people can do against this new uh,xbv-15 again funny name but uh the,number one thing people can do is make,sure they go out and get that new,bivalent vaccine,Dr John how about the covet antiviral,jug paxlovid is that still good when it,comes to being effective up against this,subvariant absolutely I mean based on,again everything we know it's a new,sub-variant we're doing the tests but we,have every reason to believe that the,oral antiviral paxilvate again super,effective drug if you get sick you,should get evaluated for getting it,um everything we know about it suggests,that it's going to work just fine,against a new variant,all right Dr ashisha thank you so much

U.S,government policy by discrediting,doctors and other experts who disagreed,and by suppressing ordinary users,including some who are sharing the cdc's,own data here to break down what he,found out journalist David swag,wonderful to have you with us David,thanks for having me all right so what,were the big headlines from your perusal,of the Twitter files what did you see,that really caught your attention,um I think there there were a few key,things,um the first one is that there certainly,was evidence of direct pressure from the,White House in both the Trump,Administration but much more so from the,Biden Administration on Twitter to get,them to moderate,um specific content,um so the first thing is government,involvement or sort of government,pressure and then the larger part of my,reporting that I found fascinating that,was that I was curious about that really,prompted me to want to go to San,Francisco was to get a little bit better,understanding of what goes on sort of,behind the scenes at Twitter,um these social media platforms that,hundreds of millions of us use it's very,very opaque about how the algorithms,work and what the process is so I really,wanted to uncover a little bit The,process by which you know they decide,which things get flagged as misleading,and which things don't,so let's talk about that process a,little bit,um you wrote that part of the issue is,that so much of the content moderation,was being done by Bots who were,insufficiently nuanced to actually,capture what even the intention was by,the people at Twitter and at other,points you you talk about content,moderation decisions that were made kind,of contrary to Twitter's own policy can,you unpack that a little bit,yeah it's it's it's pretty fascinating,um I think a lot of us hear the word,algorithm or Bots it's not really,entirely clear what that means I spent a,long time talking with an engineer at,Twitter as well as another executive,trying to just a little get a little bit,better understanding of what that stuff,means and basically when we talk about,Bots it's they they essentially train,the the employees will train the the,software the system through machine,learning and through AI they feed it you,know a handful of tweets or key terms or,other things like that and then the more,the the sort of bots crawl across the,network the more they train themselves,about what to look for and then,periodically over time humans will,actually give it more feeding it more,inputs to help sort of refine what the,Bots are looking for so that's kind of,the very basic sketch about how these,Bots work but we have to understand it,always freeze comes back to the humans,who are actually deciding what the,inputs are for the Bots to look for,about the Twitter files in the context,of the FBI and and the hunter Biden,laptop and election stuff,um I I what we saw I think at first at,least was a lot of resistance on,Twitter's part to be overly compliant,with the government and then that kind,of resistance being eroded gradually,when when the government employees keep,coming back to them over and over again,and and really you know not threatening,using very careful language but uh but,pushing them more and more toward toward,a compliance mindset,um did you see a similar thing happening,here from the White House and the CDC,perspective with respect to covid,well what we do know is from an internal,email that I found from um from an,executive ad Twitter Lauren Culbertson,where she characterized,um the multiple meetings from the Biden,White House and she characterized the,the tone of the White House staff,members being quote very angry with,Twitter that they were not being more,compliant with um the White House's you,know wishes or you know or demands as it,were,um that they really really wanted them,to be much more aggressive with,de-platforming what they whoever they,deemed were quote anti-vaxxers or,information that they deemed to be,insufficiently in in accordance with um,the sort of White House and CDC,guidelines,yeah I I want to be specific about the,kind of tweets that we're talking about,being censored here you mentioned one in,particular,um by Martin uh kuldroff who tweeted out,that basically thinking about everyone,must be vaccinated is scientifically,flawed thinking uh as as is the thinking,that nobody should be vaccinated,vaccines are more important for older,high-risk people and their caretakers,those with natural prior infection do,not need it nor children and this is,something that is obviously much more,discussed today as we have learned more,perhaps people already knew more about,uh the failures of the vaccines to,prevent transmission the way that we,thought they might before so the,argument in hospitals both for kids and for adults,are very stressed right now and it's a,combination of really of all these,viruses lots of very sick patients lots,of patients on the ventilators and the,ICU our overall Hospital capacity is,well over 100 percent,the triple democ of coveted flu and RSV,is hitting Americans and filling,hospitals as millions are expected to,travel this holiday season infections,are on the rise in many parts of the,country White House coveted response,coordinator Dr Ashish Shah joins me now,to discuss the latest good to see you as,always Dr jaw good morning Martha more,than 40 percent increased since,Thanksgiving in coveted cases and,hospitalizations that is a very big,spike it's a it's a clear increase it's,a substantial increase we're seeing it,across 90 percent of the country so yeah,covet cases are rising across the,country right now and there was a,statistic that really stood out to me,from from your White House briefing less,than half of nursing home residents have,gotten their updated coven coveted,vaccine even though deaths are occurring,mostly concentrated in people over 65,you say you're talking to leadership,trying to get them vaccinated why hasn't,this been happening yeah so um you know,there's people are still I think,learning about the new covet vaccine the,updated booster there's so a lot of,people are confused about whether they,need one or not we're being very clear,about this if you've not gotten a,vaccine in the last six months it is,essential to go out and get the new,updated uh bivalent this is an ongoing,this is ongoing work but I actually,think we're seeing real increases in the,last couple of weeks and my hope is that,that momentum continues and why this 40,percent increase is it no masks is it no,vaccines what what do you say about that,well so we're not totally surprised by,this right because we've seen increases,each of the last two Winters and then,what else is happening in colder uh,drier air the virus spreads more,efficiently people are gathering as they,should because it's really important to,gather with family and friends,um and so that combination is what is,driving the increase the the good news,here is that we can prevent those,infections from turning into serious,illness if people go out and get that,updated by valent vaccine the updated,vaccine is essential for keeping people,out of the hospital so we're making the,case that it's we're at a point where,it's safe to gather but you still have,things to do if we don't do those things,obviously things can get much worse okay,Doctor we've talked to you many times,you say the White House strategy is to,get boosted get tested wear masks you,and I have both talked about the lack of,masks across the country just not seeing,them that's the strategy but only 14,percent of people who can get that,booster have done it so how can you say,that is really working well what I would,say is it is a it is a comprehensive,strategy testing treatment uh vaccines,our job is to make sure those things are,widely available easily accessible they,remain free for Americans we're going to,continue making that case and then,obviously a lot of our efforts are,focused on older Americans those are the,ones who really tend to get sick end up,in the hospital this is ongoing work,it's shared Collective responsibilities,it's not just the White House we're,asking Governors to step up Mayors to,step up religious leaders to step up,because if we're going to get a country,as big and diverse as ours through this,difficult period we're all going to have,to pull on this together according to a,new poll from the Kaiser Family,Foundation the top two reasons people,are not getting vaccinated are number,one they don't think they need it or,they don't think the benefit is worth it,yeah so first of all from a benefit,point of view these are incredibly safe,vaccines they clearly work they clearly,reduce infections to some degree,hospitalizations and deaths to a very,very large degree you know the the thing,I remind people people often say well I,got my booster last year do I really,need it and I say well I got my flu shot,last year I don't expect that to protect,me this winter I go out and get my flu,shot every winter and the same way,people have to go out and get their,covet shot every week and I want to talk,about some anecdotal evidence about the,lack of medicines like Children's,Tylenol amoxicillin you say the supply,is out there manufacturing is out there,and you're going to work to get that,Supply into pharmacies how do you do,that are people hoarding no so here's,what's happening we constantly FDA the,health department constantly track,Supply making sure that manufacturing is,going well that the distribution is,going well right now manufacturing is,going 24 7. hello and welcome to a health policy,but things like accountable care,organizations,and bundle payments,they're not home runs but they're a,bunch of singles and doubles and maybe,you put enough singles and doubles,together and all of a sudden start,scoring and start getting in the right,direction i'm your host alan weil,when we launched a health policy a year,ago our goal was to take listeners,beyond the research published in health,affairs through conversations with our,journal authors and based on your,reaction we believe we've created,something of unique value,as we approached the one-year,anniversary of the podcast i thought who,could we invite onto the show who,captures what the podcast is all about,and the answer was easy,i invited and he accepted that's dr,ashish,who you'll hear from today,dr cha,epitomizes so much of what brings people,into health policy and health services,research and i'm really looking forward,to our conversation today he's widely,published including of course in health,affairs but his public voice arises from,a combination of deep expertise and this,unique ability to explain very complex,concepts in language accessible to the,public i have seen him,summarize a broad body of research for,an expert audience and i have read his,short twitter threads that give us more,insight into what's going on with covet,19 than pretty much any other source you,can find it is that range that uh i,think brings out or shows so much of of,his strength now i met ashish when he,was at the harvard t.h chan school of,public health we had some wonderful,conversations as he became director of,harvard global health institute,he's now the dean of the brown school of,public health and a professor of health,services policy and practice you've,probably seen him on one of his many,appearances in the news discussing,covid19 and how our nation has responded,but we're going to cover many topics,other than that,befitting someone whose experiences are,as broad as his,ashish dr ja welcome to the program,alan thank you so much for having me,here i'm really excited to spend some,time with you it's so great that you uh,agreed to join me,um you know i don't want to start with,covid although you have gained such an,appropriately gained such a reputation,as a a voice of of sanity uh in the,discussions,around how our nation has responded to,covid i want to go back to one of the,first conversations i had with you which,was actually about payment reform,we were talking about what works and,what doesn't in payment reform to try to,achieve better value in the health care,system i feel like so many of these,topics have been lost,due to,of course the important focus on the,covid pandemic but let's start with,something other than covid,go back to,those days when we were thinking about,how do we make the health care system,more efficient more effective what are,your thoughts about what it would take,to move us in that direction,yeah and,let me start off by saying i'm delighted,to spend a couple of minutes not talking,about covenant,no hey look it's very very important,obviously the most important,health crisis our country has faced in a,century and and and so spending a lot of,time talking about it is not a problem,but,um you know this health system has,really suffered through a lot the last,year and a half as a result of cobit,but the fundamental issues that animated,us a year and a half ago,um still are there and will continue to,be a problem and a challenge um,you know we're at about four trillion,dollars a year of spending on health,care which is just an enormous amount of,money,in many ways the health system performs,admirably and in lots of ways it doesn't,and i think in the conversations that we,were having before the pandemic hit the,ones that we absolutely need to have,uh every bit as vigorously and maybe,even more so and i can make the case,that maybe more so,is can we do this more efficiently can,we do this in a way,that's less taxing on individual uh,pocketbooks and on our government and on,businesses,and the two sources of greater,efficiencies in my mind that i think i,had settled on based on what i saw the,evidence,um was one was we needed to figure out,ways to bring prices down or at least,control the spur the cost they're the,high prices of health care and to slow,their their growth,and that could be everything from more,competition to moving care to lower,price point services,um but certainly consolidation in the,marketplace was not a helpful force in,the in the price,conversation more broadly there may be,individual instances where that might or,might not be true,and then the second where payment reform,has really been,at the heart and that's really when you,think about cms and medicare's efforts,medicare has fixed prices so they they,can do less on pricing,but what they have done as you know is,they've taken a whole variety of tactics,to try to reduce inefficiencies,and to me the evidence suggested and,still does

turning now to another holiday concern,large Gatherings that could add to the,so-called triple dimmick for weeks now,an alarming combination of flu covet and,RSV has been overwhelming hospitals and,made certain medicines harder to find,joining us now is Dr Ashish Shah Dr Josh,the White House's covert response,coordinator Dr Zha good to have you with,us this morning thanks for your time sir,good morning Craig thanks for having me,back it's RSV flew covid now we're,talking about an uptick in strip cases,as well as folks do gather over the next,few days socially and with families,what's your message to them what should,they know,yeah uh so the number one message is,it's really important to protect our,elderly our vulnerable and the single,best way to do that Craig is to make,sure that they are vaccinated that,everyone's vaccinated particularly older,Americans are vaccinated against both,covet and flu we have two terrific,vaccines up this year both of them,updated both of them very effective,against the circulating virus that's out,there,point though and I want to make sure I,get this right about 14.6 of the,population now uh has gotten that that,covet booster that the the White House,has been recommending why haven't more,folks you think gone out and got the,booster shot and why haven't more folks,even now going out and been vaccinated,yeah so first of all 45 million,Americans have gotten the cloven shot,that's the new bivalent booster that's,good uh you know but we need more,Americans to do it and I think there's,still uh people are learning about the,fact that we have you know moved from,getting your shot once and being done to,really needing that annual shot I don't,count on my flu shot from last year to,protect me this year same thing for,covet can't protect your Camp account on,your shot from last year to protect you,this year and just getting that message,out to Americans really important to get,that shot this year right now let's talk,about these critical medications sort of,just for a moment I was in CVS yesterday,there's they're rationing you know,Children's Tylenol and some other,medicine for kids as well I also,couldn't find Pedialyte and now I've,read that HHS Health and Human Services,we're tapping into the national,stockpile of Tamiflu I heard you say,last week that there's not a supply,issue if that's true then why are we,still rationing medicines and why are we,tapping into the national stockpile or,folks just hoarding medicine,now what we have is unprecedented demand,Craig if you think about it we're seeing,the highest levels of RSV now RSV is on,its way down so that's good news the,worst flu uh outbreak in 10 years and,then obviously covet is still here and,Rising across the country that just,means a lot of demand FDA has been in,touch with the manufacturers on a daily,basis the supplies are good they're,going 24 7. the right now the major,issues demand my hope is in the next,couple of weeks we're going to see that,demand uh temper and then we're going to,see all the stock all the shelves really,stock up full again but there's no,problem with supplies just there's a lot,of demand out there right now well I,have you before I let you go let's,quickly talk about this the CDC alert,that came out yesterday I believe,um an uptick in invasive strep cases two,children died in Colorado How concerned,should we be about this this new uh,uptick construct,yeah so this is a common childhood,infection we've seen it before often,happens after a respiratory viral,infection like RSV flu or covet,thankfully so rare here's my advice to,parents your kid's getting better let's,say they had a cold they had a,respiratory so it's starting to get,better if they start feeling worse again,really important to go and get getting,them checked out this alert from CDC was,a it was a message to doctors be on the,lookout for this treat it early all,right Dr Ashish I hope you can enjoy the,holiday weekend sir thank you,thank you hey thanks for watching don't,miss the Today show every weekday at 11,A.M Eastern 8 Pacific on our streaming,Channel today all day to watch head to,today.com all day or click the link,right here

title 42 is not consistent with our,values and it doesn't keep us safer but,keep in mind we need comprehensive,immigration reform and that's something,all the democrats are on board for,a big claim from senator elizabeth,warren as a handful of democrats are,speaking out against ending title 42,especially those democrats up for,reelection this november fox nation host,tommy lehrer joins us now so tommy,there's a huge divide,in the democratic party over this issue,you just heard elizabeth warren and,something that i thought was interesting,is the president met with the hispanic,caucus yesterday and they too,criticized title 42. on the other hand,you have about 50 democrats who want to,keep it in place so,what what's going to happen here,well listen title 42 really is the only,mechanism right now that is keeping our,country from being completely,overwhelmed and inundated although it,already really is being overwhelmed and,inundated this is really the last,barrier that's in place and when,elizabeth warren talks about it being,inhumane and not consistent with our,values i wish these democrats that are,supporting the removal of title 42 i,wish they would just come out and say we,want an open border we believe everyone,should just be able to come on in,unscreened unvetted and resettled in the,united states because that's really what,they're saying but i will say this about,title 42 eventually it will expire,eventually the cdc is going to say that,maybe covet isn't such a risk anymore,and we should all move back on with our,lives which i agree with that's why,there does need to be comprehensive,immigration reform but we need remain in,mexico and we need a process to make,sure that our country is not being,completely overwhelmed and that it's not,a free-for-all at our southern border we,need to finish the wall and certainly in,places where it has not been built or,it's sitting there rusting because the,democrats refuse to continue its,construction so we do need immigration,reform we need it to be consistent with,our values but we needed to be,consistent with the values that we have,to have a sovereign border that is,enforced first and foremost all,americans especially those that are,representing us in congress and the,white house should want that for some,reason one side or the other not really,following through on that promise to,keep america safe meantime the left,melting down over elon musk's twitter,takeover but conservatives celebrating,it as a win for free speech tommy i,would say,not just a win for free speech but for,freedom period your thoughts,i am very cautiously optimistic we're,going to have to see what elon musk does,but i will say this this is my request,to him and i think will make a big,difference i would like him to expose,all of the algorithms and censorship and,shadow banning that has gone on on,twitter especially in the last several,years i wish he would just expose it all,because i think that the american people,and especially conservatives like myself,who have dealt with social media,censorship we'd like to know what's been,done we'd like to see it all exposed,let's start there and then move forward,with actually making it a free speech,platform i think this is going to be,excellent it still remains to be seen we,hope that he will keep to his promise of,returning it to a place a marketplace of,ideas but we're gonna have to wait and,see how it shakes out i do wish again,that he would expose everything that's,gone on because the american people,deserve to know yeah what do you make of,people in the media and liberals who are,criticizing elon musk for promoting free,speech i mean these are two two groups,of people that should be champions of,freedom of speech and historically they,have been,i think the truth,terrifies democrats and those on the,left because for so long they've been,able to hide behind social media and big,tech and those that are the wardens of,social media and big tech to make sure,that they control the agenda and the,narrative so someone like elon musk,terrifies them he's not a conservative,he's not a republican he's someone that,truly does believe in free speech and,that should be celebrated by all but i,think the democrats are worried if they,aren't having such a control over big,tech and social media maybe the truth,will get out and if the truth gets out,maybe just maybe people will change the,way that they vote in november and in,elections to come and that is why,they're shaking in their boots if the,refs aren't on your side the game could,be fair and that scares them tommy thank,you very insightful with that fox and,friends starts right see ya
